What to do with your Dementia Directive.

1.  Think carefully about each question and fill it out.

2.  Then share itThe most important thing you can do is share your answers with the people who would be involved in making decisions about your care if you weren't able to.

3.  Give a copy to each of them. Tell them to save it, so they can look back for guidance about what your wishes were.

4.  Give a copy to your doctor. Ask your doctor to put a copy of it in your medical record.
